Care Coordination

Each member of Kendal at Home is assigned a personal Care Coordinator.  You and your Care Coordinator will begin a personal relationship that will continue throughout your lifetime.  He or she will become familiar with your lifestyle and preferences. The coordination of care concept means that your care plan is personalized; it will be designed by you, your care coordinator, and your physician to help you stay well and maintain your highest level of independence. Your plan will emphasize preventative care and wellness, and your care coordinator will be your direct point of contact, 24 hours a day, 7 days a week.

For Director of Care Coordination David Zientarski it's all about understanding - understanding your needs as a member, and understanding how to best use Kendal at Home's resources to meet those needs.

Whether you have the flu, have a broken hip, or need assistance on a regular basis, we will provide you with the services you need, as soon as you need them and for as long as you need them. All you have to do is contact your Care Coordinator and he or she will make sure you receive services as soon as possible.
